Dissertation hosting consent form

I give permission for the document cited above to be stored in full on the Cornwall Health Library(CHL) private folder and shared with CHL library members requesting a copy.

The document can be moved to different storage by Cornwall Health Library if needed.

The document details will be shared publicly on the Library Management System / catalogue and sometimes on social media and other comms.

Copyright remains with the author cited above.

The author will make a request in writing if they wish to have the document deleted.

The document can be duplicated for backup, distribution or migration to other storage by CHL.

That I am the author and owner of the document cited above.

For any substantial  third party material that is included in your document and under copyright, I have obtained appropriate permissions and identified the material.

The document is original, does not break any laws or other contracts.

Cornwall Health Library is not under obligation to legally defend infringement of any type within the document.
